You have the right to access your personal information.

You have the right to access your personal health information that Each holds.

Under the Health Records Act 2001 (Victoria), you can request:

  • copies of your health records
  • to view your information in person
  • a summary of your health information prepared by your provider.

How to request your health information

To access your health records:

  • complete the request form below
  • provide your personal details and what information you need
  • choose how you want to receive your information.

We'll respond to your request within 45 days.

Fees that may apply

Some fees may apply when accessing your records:

  • photocopies of your health information
  • viewing your health information
  • receiving a summary of your health record.

If you have a valid pension or concession card, these fees may be waived.

Requesting records for someone else

You can request health records for another person if you:

  • have their written permission
  • are their legal guardian or hold power of attorney
  • are the legal representative of a deceased person.

You'll need to provide evidence of your authority to act on their behalf.

Correcting your information

If you believe your health records contain information that is incorrect, incomplete, misleading or out of date, you can ask us to correct it.

Simply write to us explaining why you think the information needs changing. We'll respond to your request within 30 days.
